Do you have to strain to hear normal conversation?
Do you have to watch other people's faces to follow what they are saying?

Have you ever suffered from a trauma to the head or ear?
Is there anyone in your immediate family with hearing loss?
Have you ever been exposed to very loud noises, such as gunfire or industrial noise, without ear plugs or other noise protection?
